Introduction

From our previous engagement with families and carers, we know we need to improve the information and advice around preparing for adulthood. We call the move from children's services to Adult Social Care, Transitions. This work is part of our action plan under the preparing for adulthood programme.

From what you tell us, we can identify gaps in our information and advice offer and share what may be available already. We will need your support to tell us what improvements or changes are needed e.g. making this information easier to understand or creating new information using different formats, for example videos.

We have prepared a short questionnaire to help us start our conversation with you.

We are also holding a workshop on 3 October 2019. You'll be directed to details on how to register for this after submitting your questionnaire.

We'd like you to complete the questionnaire by Monday 30 September 2019, but if you can't, please send it back to us as soon as you can.
